About this form
This form lets payroll know how many timesheet or overtime hours your support staff have worked, and the allowances payroll should pay them.
When to use this form
Use this form:
- to pay timesheet employees for casual or fortnightly hours they have worked, or an allowance they are due
- to pay non-timesheet employees for additional hours worked or additional allowances due
- if you do not use i-paydata.
What you will need to complete the form
You will need each employee's:
- MOE number
- personal details
- dates worked
- pay rate, or time at which they are to be paid, for example ordinary time, time-and-a-half
- allowance due (if you are paying an allowance).
