When to use this form

Use this form:

• once payroll has set up relief teacher on your school's payroll
• to pay a relief teacher for the hours they have worked
• to pay a relief teacher for their travel to and from your school
• if you do not use i-paydata or e-Reliever.

What you will need to complete the form

You will need the teacher's:

• MOE number
• name
• dates and part-days worked
• travel distance to and from your school.
