This form lets payroll know future changes to teachers' units, payments or other allowances.

When to use this form

Use this form:

• once you and your teachers have formally agreed to change the unit, payment or allowance details for their positions
• if you do not use i-paydata.

What you will need to complete the form

You will need each employee's:

• MOE number
• personal details
• changed employment agreements.
