This form requests payroll to assess a teacher's new, improved or additional qualifications.

When to use this form

Use this form when a teacher has received new, additional or improved qualifications which payroll has not assessed.

What you will need to complete the form

You will need the:

• MOE number
• personal details
• IRD number (if they do not have an MOE number)
• details of current qualifications that payroll has already assessed
• certified true copies of any new/additional/improved:
• New Zealand teaching and academic qualifications
• transcripts for the Bachelor of Education or any conjoint degree
• professional and technological qualifications
• overseas qualifications and the NZQA report.
