About this form

This form requests payroll to apply a teacher's service increment.

When to use this form

Use this form for permanent secondary and area teachers who:

• either as a secondary teacher do not hold a permanent unit, or as an area teacher do not hold more than three management units
• have completed three years or more on the top step of the salary scale for their qualification group
• have a Group 4 (Honours) or Group 5 (Masters) qualification level, or they have gained an additional approved qualification.

What you will need to complete the form

You will need the:

• MOE number
• personal details
• certified true copies of any New Zealand academic qualifications the teacher lists.
