When to use this form

Use this form for a permanent or fixed-term teacher you appoint, who had previously resigned or taken leave from a permanent teaching position to care for their own child.

What you will need to complete the form

You will need the teacher's:

• MOE number
• personal details
• certified true copies of birth or adoption certificates of any children they list
• job details (for jobs they had during the period of childcare).
