About this form

This form lets payroll know that a non-teaching staff member is entitled to, and wants to have their pay annualised for the school year.

The annualisation year runs from pay period 23 of one year to pay period 22 of the following year.

When to use this form

Use this form to annualise the pay of your non-teaching staff member for the coming school year.

What you will need to complete the form

You will need the employee's:

• MOE number
• name
• job number for this position (which you can find on your staff usage and expenditure (SUE) report)
• employment agreement.
