Use this form once you and your annualised support staff member have formally agreed to permanently change the hours of work, pay rate, or first aid or qualification allowances for their position.

What you will need to complete the form

You will need the employee's:

• MOE number
• name
• job number for this position (which you can find on your staff usage and expenditure (SUE) report
• annualisation agreement for this annualisation year
• changed employment agreement.
