About this form
This form lets payroll know an annualised support staff member has stopped annualisation:
- voluntarily, or
- because they have been on leave without pay, or
- because they have resigned or ended their employment at your school.
When to use this form
Use this form at least three weeks before annualisation is to stop for a support staff member at your school.
What you will need to complete the form
You will need the employee's:
- MOE number
- name
- job number for this position (which you can find on your staff usage and expenditure (SUE) report)
- weeks they have worked this year
- number of public holiday days they are entitled to
- annualisation agreement.
You will also need a Resignation/End of Employment Form ESP6 if annualisation is stopping because the support staff member is leaving this position.
