About this form

This form is to tell payroll to pay backdated arrears owed to employees who:

• no longer work in the education sector (and are not on the payroll)
  
• were employed at the time arrears from their collective agreement apply.

When to use this form

Use this form to request payroll pays collective agreement arrears after an employee's employment has ended.

What you will need to complete the form

You will need the employee's:

• MOE number
• personal details
• IRD number
• current bank account details including a deposit slip.
