About this form

This form lets payroll know that your employee is entitled to receive a maternity grant or ex-gratia parental payment.

When to use this form

Use this form once your employee has given you their baby's:

• birth certificate, or
• approved adoption placement.

Your employee must have already been eligible to take maternity leave for this child.

What you will need to complete the form

You will need the employee's:

• MOE number
• personal details
• baby's birth certificate or approved adoption placement.
