About this form
This form lets payroll know:
- who is joining KiwiSaver (by auto enrolment or opting in)
- if your employee is already an existing KiwiSaver member and the date their membership started
- the employee's date of birth for KiwiSaver purposes
- whether your employee is a member of another state sector retirement savings scheme.
When to use this form
Use this form to collect information from your employee to:
- assess if you need to auto enrol them in KiwiSaver, or
- assess if they are eligible to opt in (if they choose to join KiwiSaver by opting in), or
- find out if they are already a KiwiSaver member.
What you will need to complete the form
You will need the employee's:
- MOE number
- personal details
- employment agreement
- identification, for example a passport, that verifies their date of birth and whether they are a New Zealand citizen.
