When to use this form
Use this form:
- once you and your support staff member have formally agreed to change the pay details for their position
- if you do not use i-paydata.
What you will need to complete the form
You will need the support staff member's:
- MOE number
- personal details
- job number for this position (which you can find on your staff usage and expenditure (SUE) report)
- changed employment agreement.
