This form lets your pay centre know which of your school's payroll administrators should receive payroll reports by email.

When to use this form

Use this form to:

• sign-up to receive your school's reports for e-SUE, e-Leave, and e-Increments (for your teachers)
• change the payroll administrator who receives reports for e-SUE, e-Leave and e-Increments.

What you will need to complete the form

You will need:

• the name and email address of each payroll administrator who is to receive your school's reports for e-SUE, e-Leave and e-Increments
• your school's elected password for this electronic service.
