About this form
This form lets payroll know:
- that your employee's job or position has ended
- when to stop paying your employee.
When to use this form
Use this form:
- when an employee resigns from a position
- when an employee leaves a position
- when an employee finishes a position or job
- if you do not use i-paydata.
Use a separate form for each position the employee finishes. For example, if your employee works in two separate jobs and resigns from your school, complete a form for each job.
What you will need to complete the form
You will need the employee's:
- MOE number
- personal details
- job number for this position (you can find this on your staff usage and expenditure (SUE) report)
- leave balance (for non-teaching employees)
- hours they worked in the last fortnight (if they are a part-time support staff member).
You will also need:
- the date the employee will be last working in that position
- written ministry approval for medical retirement (if they are medically retiring).
