About this form

This form requests payroll to assess a teacher's salary.

When to use this form

Use this form when a teacher:

• is new to teaching in a state or state-integrated school, in other words this is their first position
• has not taught in a state or state-integrated school for two years.

What you will need to complete the form

You will need the teacher's:

• MOE number (if they have been paid through the education service payroll before)
• personal details
• IRD number (if they do not have an MOE number)
• certified true copies of:
• New Zealand teaching and academic qualifications
• transcripts for the Bachelor of Education or any conjoint degree
• incomplete qualifications you list ie a certified copy of the transcript, and a letter from the University Registrar confirming you have (and the date you) completed
  2/3rds of a degree without any cross credits from the Teaching Diploma
• professional and technological qualifications
• overseas qualifications and the NZQA report
• statements of service for all schools other than NZ state or NZ state-integrated schools
• relevant work experience documentation.
